Job summary

Premier System Support is seeking a Chart Completion Tech to preps, scans, and validate paper documents using an optical scanning system to ensure complete and accurate medical records. The role includes reviewing charts for documentation deficiencies and releasing information for continuity of care.

The position requires at least a high school diploma and a course in medical terminology, with 1–3 years in a medical office setting.

Qualifications

• Must have completed a course in medical terminology or be enrolled in a class.
• High school completion or GED required.
• 1–3 years of job-related experience in a medical office.
• Preferred experience in Health Information or related medical field; knowledge of Word & Excel.

Responsibilities

• Prep, scan, and validate all paper documents into the optical scanning system.
• Scan and validate loose reports sent to the department.
• Analyze medical records for deficiencies in chart documentation.
• Handle release of information for continuity of care requests.
• Ensure HIPAA guidelines are followed to reduce liability in releasing records.
• Process government audit requests and specialized chart reviews (UVMC only).
• Monitor incomplete charts and follow up with physicians.
• Follow JC and Conditions of Participations guidelines and all federal and state regulations.
