Job summary

Adelante Healthcare seeks a Medical Records Representative in Phoenix to manage scanning of patient consultations, lab and diagnostic reports, and other documents into charts. This role handles outgoing requests, processes records from hospital portals, and supports legal requests for records while ensuring accuracy and privacy.

The position requires 1–2 years in medical records, 1–2 years in customer service, basic medical terminology, and strong organization.

Qualifications

• Ability to communicate effectively with patients, co-workers, and the public via in person, written and phone channels.
• 1–2 years of scanning and medical records experience plus 1–2 years of customer service experience.
• Basic knowledge of medical terminology and high accuracy in handling records.
• Strong organization skills and ability to prioritize multiple tasks simultaneously.
• Ability to work independently and as part of a team; proficient typing and office skills.

Responsibilities

• Sort daily incoming faxes and scan documents into patient charts accurately.
• Research and verify information within patient charts and respond to record requests.
• Process incoming medical records from hospital portals and handle external requests.
• Maintain production standards and communicate with office staff and providers as needed.
• Support cross-training and adapt to new skills and responsibilities.
• Uphold compliance with policies and protect patient privacy.
