Job summary

Qualifications

Responsibilities

• Assist providers and clinical teams with patient care.
• Maintain patient records and documentation in EHR.
• Complete administrative duties to support patient care management.
• Field patient calls and voicemails promptly and professionally.
• Address patient portal and EHR messages within established timeframes.
• Ensure accuracy and proper documentation in the patient’s EHR.
• Communicate with the clinical care team as needed.
