We are looking for a Care Coordinator - Medical Assistant to help with the way primary care is delivered and how patients are served. The Care Coordinator facilitates the delivery of care by the Provider. Care Coordinators are liaisons to the patient/family/caregiver working with the Provider's direction and orders to provide quality healthcare for the homebound or home-limited patients.
This role involves in-home, assisted living, and independent-living community-based work, requiring frequent travel in various weather conditions. Work settings include conditioned air, artificial lighting, and open workspaces. You must be able to travel frequently by car or public transportation, communicate effectively, and occasionally lift up to 50 lbs. Good manual dexterity and reasoning skills are essential, along with the ability to understand and utilize reports and documents.
Compensation: From $18.00 to $23.00 per hour.
