Job summary

Upstate Caring Partners is seeking a Broker to assist individuals and their supports in developing and maintaining Self-Directed Plans that stay current for Medicaid funding.

You will develop budgets, educate families, review expenditures, and assist with staffing and community connections, ensuring continuity and crisis prevention. Travel is required; NYS driver’s license is necessary.

Qualifications

• Bachelor’s degree in a related field (Psychology, Sociology, Human Services) is required.
• Minimum 1 year experience working with developmental disabilities.
• Must meet OPWDD Broker training and certification requirements.
• Knowledge of local community resources is required.
• Travel is required; valid NYS driver’s license is required.

Responsibilities

• Develop and maintain the Self-Directed Plan/Budget with quarterly reviews.
• Provide education, training, and support to individual and family for the Self-Directed plan/budget.
• Review expenditures and assist with budgeting and monthly spending detail.
• Assist in recruiting, interviewing, hiring and supervising support staff.
• Support building a positive circle of supports and community connections.
• Problem-solve to prevent crises and preserve Medicaid funding.
• Collaborate with internal and external programs for the individual’s best interests.
• Provide crisis support which may require off-hours/weekend intervention.
