Job summary

Indiana Professional Management Group, Inc. (IPMG) is seeking a full-time Case Manager to serve Lawrence County, Indiana.

The role is hybrid, combining remote work from your home with in-person community meetings up to several times a week; travel up to about an hour within the county is required, and applicants must reside in Lawrence County. You will provide information on Medicaid waiver processes, develop Individual Support Plans based on each Person's strengths, needs, goals, and desires,

Qualifications

• A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Or a Bachelor's degree in an unrelated field with a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
• Valid driver's license, car insurance, and reliable transportation.
• The ability to pass thorough background screens.
• Designated home office setting with computer, internet service, and smart phone.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

Education
