Job summary

Indiana Professional Management Group, Inc. is seeking a Case Manager to serve Hendricks County, Indiana. The role is full-time and hybrid, combining remote work from a home office with travel to community meetings several times a week within Hendricks County and surrounding areas.

You will provide information about the Medicaid waiver process, develop individualized support plans, help clients access needed services, and facilitate team meetings with providers to ensure coordinated care.

Qualifications

• Bachelor's degree in human services or related field.
• Bachelor's degree in an unrelated field with 1+ year direct experience with individuals with disabilities or aging population.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass thorough background screens.
• Dedicated home office with computer, internet, and smartphone.

Responsibilities

• Provide information and education about Medicaid waiver process.
• Create an Individual support plan based on clients' strengths, needs, goals, and desires.
• Help individuals access services and supports to meet their goals.
• Facilitate regular community-based team meetings to review plans of care and coordinate with providers.
