Job summary

Indiana Professional Management Group, Inc is seeking a Case Manager for a full-time hybrid role serving Greene County, Indiana. The position combines remote work with local travel for community-based meetings several times a week; residency within Greene County is required.

Responsibilities include educating clients about Medicaid waivers, creating individualized support plans, helping access needed services, and coordinating with teams and providers to review plans of care.

Qualifications

• Bachelor's degree in human services fields or related experience.
• Alternative: Bachelor's in an unrelated field with 1 year direct experience with individuals with intellectual/developmental disabilities, or 2 years with aging population.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve.
• Help Individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.

Education

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation or another human services field
