Job summary

Indiana Professional Management Group, Inc. is seeking a full-time Case Manager for a hybrid role serving Lawrence County, Indiana. Residents must live in Lawrence County and be willing to travel within the county for in-person meetings.

You will educate individuals about Medicaid waiver options, build individualized support plans, coordinate services, and lead community meetings with providers.

Qualifications

• A Bachelor's degree in psychology, sociology, social work, or related human services field.
• Or Bachelor's in an unrelated field with relevant direct experience in disability services or aging population.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass background screens.

Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an Individual support plan based on the strengths, needs, goals, and desires of the individuals we serve.
• Help individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.

Education

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field

Bachelor's degree in unrelated field + 1 year direct experience with individuals with intellectual/developmental disabilities or 2 years aging population
