Job summary

Indiana Professional Management Group, Inc. (IPMG) is seeking a Case Manager for a full-time hybrid role serving Henry County, Indiana.

The position combines remote work from your home with travel to community meetings within the county. You will educate individuals about Medicaid waivers, create individualized support plans, help them access needed services, and coordinate with providers through regular team meetings.

Qualifications

• Bachelor’s degree in psychology, sociology, social work, or related field.
• Alternative: non-human services degree with 1 year direct experience or 2 years with aging population.
• Valid driver’s license, car insurance, and reliable transportation.
• Ability to pass thorough background screens and set up a home office.

Responsibilities

• Provide information to help individuals understand Medicaid waiver processes.
• Create an Individual support plan based on strengths, needs, goals, and desires of individuals served.
• Help individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plans of care and collaborate with providers.
