Outfield Healthcare Partners is seeking a seasoned MDS Coordinator in Houston, TX to manage RAI and care management from admission to discharge. You will work under the Director of Nursing and VP of Clinical Reimbursement to ensure regulatory compliance and accurate MDS coding.

The role requires RN or LVN licensure, at least two years in a Skilled Nursing Facility, and familiarity with Case-Mix, Medicare PPS, and Medicaid reimbursement.

Formación

• RN or LVN licensed in the state of practice.
• Minimum 2 years nursing experience in a Skilled Nursing Facility.
• Thorough knowledge of Case-Mix, Medicare PPS and Medicaid reimbursement.
• Understanding of OBRA regulations and the MDS process.
• Experience with MDS 3.0 preferred.

Responsabilidades

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments.
• Monitor and document MDS and care plans to support coding accuracy.
• Coordinate admission, assessment reference date setting and discharge processes.
• Ensure compliance with federal/state regulations for RAI processes.
• Maintain up-to-date MDS status per guidelines and document in medical records.
