Job summary

PACS Group is seeking a Nurse Manager with a strong background in MDS/PDPM processes to oversee facility assessments and care planning. The role focuses on supervising the MDS workflow, ensuring timely and accurate documentation, and coordinating interdisciplinary teams to meet regulatory standards.

The ideal candidate will hold a BSN, maintain state licensure, and bring 3 years of supervisory experience in long-term care, with proficiency in Microsoft Office and PCC software for data management.

Qualifications

• Requires BSN; state board in good standing; continuing education/licensing requirements.
• Minimum 3 years supervisory experience; long-term care experience preferred.
• Must be proficient with Microsoft Office Suite and data reporting tools.

Responsibilities

• Oversee facilities’ weekly PDPM meetings for skilled patients.
• Monitor PDPM tools usage and ensure accurate data entry and submission.
• Provide PDPM and MDS training to new MDS Coordinators.
• Assess health status and level of care for new admissions.
• Ensure timely completion of all MDS assessments (PPS Medicare, quarterly, annual, significant change).
• Coordinate MDS completion with interdisciplinary teams within regulatory timeframes.
