Job summary

PACS is seeking an experienced Nursing Supervisor to oversee the facility’s resident assessment processes, including MDS and PDPM oversight. You will coordinate interdisciplinary teams, ensure accurate assessments, and guide care planning and documentation for residents.

The role emphasizes leadership in MDS completion, regulatory compliance, and data-driven improvement of care outcomes within a long-term care setting.

Qualifications

• Graduation from an Accredited School of Nursing with a Bachelor of Science Degree in Nursing preferred.
• Must be in good standing with the State Board of Nursing and maintain required continuing education/licensing.
• 3 years of supervisory experience preferred. Long Term Care Experience Preferred.

Responsibilities

• Oversee facility weekly PDPM meetings for skilled patients.
• Track and monitor PDPM tools and utilization.
• Monitor QM and QASP performance and provide data-driven clinical recommendations.
• Provide PDPM and MDS training to new MDS Coordinators.
• Assess health status and level of care of new admissions.
• Ensure timely completion of MDS Assessments including PPS Medicare and annual reviews.
• Coordinate MDS with interdisciplinary teams and ensure proper documentation.
• Schedule and conduct resident care conferences in compliance with regulations.
• Assist in formulating and revising care plans to meet resident needs.
• Maintain confidentiality of all resident information.
