Job summary

Trellis Paradise is seeking a Nursing Manager to oversee the facility's PDPM and MDS processes and ensure the resident assessment plan of care is implemented effectively.

The role focuses on leading MDS coordination, keeping documentation compliant, and guiding staff in PDPM-related activities, with a strong emphasis on quality outcomes in long-term care. This on-site role requires nursing leadership and regulatory acumen.

Qualifications

• Graduation from an Accredited School of Nursing with a Bachelor of Science Degree in Nursing preferred.
• Must be in good standing with the State Board of Nursing and maintain all required continuing education/licensing requirements.
• 3 years of supervisory experience preferred.
• Long Term Care Experience Preferred.

Responsibilities

• Oversee the facility's weekly PDPM meetings for skilled patients.
• Track and monitor facility utilization of PDPM tools and related documentation.
• Monitor QM and QASP performance and provide data analysis and clinical recommendations.
• Provide PDPM and MDS training to new MDS Coordinators.
• Assess and determine health status and level of care for new admissions.
• Ensure timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, and significant change.
• Coordinate interdisciplinary participation in completing the MDS for each resident.
• Maintain an accurate schedule of all MDS assessments and ensure proper reference dates.
• Enter and submit MDS data accurately; verify submissions and maintain records.
• Coordinate and conduct resident care conferences in compliance with regulations and complete MDS reviews.
• Assist disciplines in formulating and revising care plans and ensure appropriate nursing interventions.
