Job summary

PAC in Salt Lake City, UT is seeking an experienced MDS/PAC leader to oversee resident assessments, PDPM planning, and care coordination. You will drive MDS accuracy, coordinate with RDCS, and ensure timely submissions.

The role requires a BSN (or higher), an active RN license, and at least 3 years of supervisory experience in long-term care. You will facilitate monthly PDPM workshops, develop reporting tools, and mentor MDS nurses.

Qualifications

• BSN preferred; active RN license required.
• Minimum 3 years of supervisory experience in long-term care.
• Strong knowledge of MDS processes and CMS PDPM regulations.
• Ability to analyze data and prepare actionable reports.
• Experience delivering staff trainings on regulatory topics.

Responsibilities

• Oversee facilities' weekly PDPM meetings and utilization of PDPM tools.
• Create and maintain clinical reporting systems between RDO and RDCS.
• Hold monthly regional MDS workshops on PDPM and CMS regulations.
• Monitor QM and QASP performance; provide data-driven recommendations.
• Review MDS/PDPM claims for accuracy and trend analysis with RDCS.
• Develop and deliver ongoing PDPM/MDS trainings for new coordinators.
