Optalis Healthcare seeks an experienced RN MDS Nurse to coordinate and complete Minimum Data Set assessments in compliance with CMS and state regulations. You will assure accurate resident assessments, care planning, and quality care while collaborating with interdisciplinary teams.

Qualificações

• Must hold an active RN license in the specified state.
• Experience with MDS coordination is preferred.
• Strong knowledge of CMS guidelines, RAI, and care planning required.
• Excellent attention to detail and organizational skills.
• Ability to collaborate with clinical and administrative teams.
• Proficiency with electronic health records (EHR).

Responsabilidades

• Complete and oversee MDS assessments (admission, quarterly, annual, and significant change).
• Ensure CMS guidelines and state/federal regulations compliance.
• Coordinate care planning with interdisciplinary team members.
• Review medical records for accuracy and completeness.
• Monitor quality measures and reimbursement optimization.
• Participate in audits and implement corrective actions as needed.
• Educate staff on documentation and MDS-related processes.
