Overlook Skilled Nursing & Rehabilitation is seeking a highly organized MDS Coordinator to ensure accuracy of MDS assessments and provide support to our care team. The role emphasizes timely data verification and adherence to Medicare guidelines in a busy clinical setting.

The ideal candidate will have at least 2 years of MDS coordination experience, an active RN license, strong analytical and communication skills, and familiarity with MDS software and EHR systems.

Qualifications

• Minimum 2 years of experience in MDS coordination or a related field.
• RN license is required.
• Strong analytical and problem-solving skills.
• Excellent communication and interpersonal skills.
• Ability to work in a fast-paced environment with multiple priorities.
• Familiarity with MDS software and electronic health records (EHRs).

Responsibilities

• Coordinate the collection and outcome of MDS assessments for all residents, ensuring compliance with Medicare guidelines and regulatory requirements.
• Audit and verify the accuracy of MDS data, making necessary corrections and updates in a timely manner.
• Collaborate with healthcare professionals to obtain necessary information and complete MDS assessments.
• Provide education and support to clinical staff on MDS assessment processes and guidelines.
• Participate in quality improvement initiatives to enhance the accuracy and completeness of MDS assessments.
• Develop and maintain organized systems for tracking and storing MDS documentation.
