Job summary
Life Care Centers of America is seeking an LPN MDS Nurse to accurately complete MDS Assessments, CCAs, and Care Plans in compliance with CMS RAI Manual regulations. The role requires current LPN/LVN licensure and a minimum of two years’ nursing experience, preferably in geriatric care.
You will report changes to the DON, educate staff on CMS updates, and participate in interdisciplinary care plan reviews. CPR must be current; strong confidentiality and adherence to policy are essential.

- Advanced nursing knowledge and clinical skills.
- Ability to interpret CMS RAI manuals and related regulations.
- Strong communication within an interdisciplinary team.
Responsibilities
- Complete MDS Assessments, CCAs, and Care Plans accurately and timely.
- Notify DON of changes in patient condition identified by MDS.
- Educate direct care staff on CMS RAI updates affecting documentation.
- Assist with Interdisciplinary Comprehensive Care Plan reviews.
- Review Final Validation Reports and CMS QIES submissions before billing.
- Attend PPS and other meetings; function as staff nurse when needed.
- Maintain patient rights, safety, and sanitation standards.
- Demonstrate reliability, professionalism, and good attendance.
