Life Care Centers of America is seeking an LPN MDS Nurse to ensure accurate MDS Assessments, CCAs, and Care Plans in line with CMS RAI Manual.

You will collaborate with the Interdisciplinary team, educate staff on CMS updates, and ensure timely submissions while upholding the facility's standards and patient rights. This is an on-site role in Tennessee with opportunities for professional growth and quality resident care.

Qualifications

• Graduate of an accredited LPN program.
• Active LPN/LVN license in good standing.
• Two years nursing experience; geriatric experience preferred.
• CPR certification required.

Responsabilités

• Complete and submit MDS Assessments, CCAs, and Care Plans per CMS RAI Manual.
• Notify DON of changes in a patient’s condition identified by the MDS Assessment.
• Educate direct care staff on CMS RAI updates and facility regulations.
• Assist with review of the Interdisciplinary Comprehensive Care Plan.
• Review Final Validation Reports and CMS QIES submissions before billing.
• Attend PPS meetings and other required meetings; perform staff nurse duties as needed.
• Maintain patient rights and safety, and uphold sanitation standards.
• Maintain confidentiality and adhere to Life Care’s policies and Code of Conduct.
• Communicate effectively on interdisciplinary teams.
