Job summary

Life Care Centers of America is seeking a dedicated LPN MDS Nurse to accurately complete and submit MDS Assessments, CCAs, and Care Plans in accordance with CMS RAI manual regulations. The role requires active LPN/LVN licensure, at least two years of nursing experience (geriatrics preferred), and CPR certification.

Responsibilities include educating staff on CMS updates, reviewing interdisciplinary plans, and ensuring timely CMS reporting.

Qualifications

Attended an accredited LPN program.
Currently licensed in applicable State with active LPN/LVN license.
Two years nursing experience; geriatric experience preferred.
CPR certification upon hire or during orientation; must remain current.

Responsibilities

Complete and submit accurate MDS Assessments, CCAs, and Care Plans per CMS RAI manual.
Report changes in a patient's condition to the DON.
Educate direct care staff on CMS RAI updates that impact documentation.
Review the Interdisciplinary Comprehensive Care Plan.
Review CMS Reports to identify late assessments and develop prevention systems.
Attend PPS meetings and other required meetings.
Perform staff nurse duties as needed.
Provide excellent customer service and maintain professionalism.
Assist in patient evacuation as needed.
Maintain dependable attendance and teamwork.
Communicate effectively on an interdisciplinary team.
