Job summary

Life Care Centers of America in Chesterfield, MO is seeking an LPN MDS Nurse to complete and submit accurate MDS Assessments, CCAs, and Care Plans in compliance with CMS RAI manual regulations.

The ideal candidate has an accredited LPN program, an active LPN/LVN license in good standing, and at least two years of nursing experience (geriatric experience preferred). CPR certification is required at hire or during orientation and must remain current.

Qualifications

• Attended an accredited LPN program.
• Licensed/registered LPN/LVN in the applicable state in good standing.
• Two (2) years’ nursing experience; geriatric experience preferred.
• CPR certification at hire or during orientation and kept current.

Responsibilities

• Complete and submit MDS Assessments, CCAs, and Care Plans per CMS RAI guidelines.
• Report changes in a patient’s condition identified by the MDS to the DON.
• Educate direct care staff on CMS RAI updates affecting documentation.
• Assist with interdisciplinary care plan review.
• Review CMS Reports to identify late assessments and improve processes.
• Attend Daily PPS Meeting, Monthly Triple Check, and other meetings as requested.
• Perform staff nurse functions when needed.
• Demonstrate excellent patient service and professionalism.
