Job summary

Life Care Centers of America is seeking an LPN MDS Nurse to complete and submit accurate MDS Assessments, CCAs, and Care Plans in accordance with CMS RAI Manual regulations.

The role requires an active LPN license, two years of nursing experience (geriatric preferred), and CPR certification. A $5,000 sign-on bonus is offered. Responsibilities include multidisciplinary coordination and ensuring timely CMS reporting.

Qualifications

• Attended an accredited LPN program.
• Currently licensed/registered in applicable State with an active LPN/LVN license in good standing.
• Two (2) years’ nursing experience; geriatric nursing preferred.
• CPR certification upon hire or during orientation and kept current.

Responsibilities

• Complete and submit accurate MDS assessments, CCAs, and care plans per CMS RAI manual regulations.
• Report changes in a patient’s condition identified by MDS to the DON.
• Educate direct care staff on CMS RAI changes impacting documentation.
• Assist with review of Interdisciplinary Comprehensive Care Plan.
• Review CMS reports and ensure timely submissions; notify Business Office if not ready to bill.
• Attend PPS and other required meetings and support RN MDS Coordinator as needed.
• Perform staff nurse duties and maintain excellent patient service.
