Job summary

PACS is seeking an experienced RN Supervisor/MDS Nurse for a dual role at a skilled nursing facility. The position manages resident assessments, MDS processes, and care planning during the tour of duty.

Responsibilities include leading PDPM meetings, ensuring timely MDS submissions, coaching coordinators, and maintaining compliance with federal and state regulations; part-time split duties.

Qualifications

• BSN or equivalent nursing degree.
• Active RN license in good standing.
• 2+ years of long-term care supervisory experience.
• Experience with EHR PCC preferred.

Responsibilities

• Oversee the facility's weekly PDPM meetings for skilled patients.
• Monitor PDPM tools usage and data for clinical recommendations.
• Provide PDPM and MDS training to new coordinators.
• Ensure timely completion of all MDS assessments including PPS Medicare, quarterly and annual reviews.
• Coordinate interdisciplinary participation in completing the MDS for new admissions.
