Job summary

PACS is seeking a nursing leader to oversee the facility-wide resident assessment process, including MDS and PDPM activities. You will manage assessments, coordinate interdisciplinary teams, and ensure regulatory compliance for new admissions and ongoing care planning.

The role requires BSN, active State Board of Nursing status, and preferred supervisory and LTC experience. Proficiency with Microsoft Office and PCC is advantageous; expect a data-driven environment and regular KPI reviews.

Qualifications

• Graduation from an accredited nursing program with a BSN preferred.
• Must be in good standing with the State Board of Nursing and maintain CE requirements.
• 3 years of supervisory experience preferred.
• Long Term Care experience preferred.
• Proficient in Microsoft Office; PCC knowledge preferred.

Responsibilities

• Oversee facility-wide PDPM meetings for skilled patients.
• Monitor use of PDPM tools and CMS documentation workflows.
• Provide PDPM and MDS training to new MDS Coordinators.
• Assess health status and level of care for new admissions.
• Ensure timely completion of MDS assessments and PPS/annual reviews.
• Coordinate interdisciplinary MDS participation and regulatory documentation.
