Job summary

PACS Group, Inc. is seeking an experienced MDS Coordinator to oversee the facility's MDS processes and ensure assessments are completed per guidelines.

The role emphasizes resident assessments, MDS submissions, and a collaborative care planning approach. You will lead PDPM meetings, track performance, provide training to new coordinators, and coordinate interdisciplinary input to ensure accurate level of care determinations and timely documentation.

Qualifications

• Graduation from an Accredited School of Nursing with a BSN preferred.
• Must be in good standing with the State Board of Nursing and maintain continuing education/licensing.
• Minimum 3 years of supervisory experience preferred.
• Long Term Care experience preferred.
• Knowledge of MDS/PDPM processes and healthcare regulations.

Responsibilities

• Oversee facility MDS processes and ensure assessments meet guidelines.
• Lead weekly PDPM meetings and track MDS-related performance.
• Provide PDPM and MDS training to new MDS Coordinators.
• Coordinate multidisciplinary input for MDS completion and care plans.
• Ensure timely submission of MDS assessments and maintain documentation.
• Maintain confidentiality of resident data.
