PACS is seeking an MDS Coordinator – RN to oversee the MDS/PPS program and ensure timely completion of assessments across departments. You will develop coordinated nursing care plans, analyze complex data, and drive compliant documentation within regulatory timelines.

The role requires an active RN license, MDS/RAI experience, strong clinical assessment, and excellent communication skills. Knowledge of Medicare guidelines and CMG classifications is a plus.

Qualifications

• RN program completed and active RN license in good standing.
• Experience in MDS coordination and RAI process preferred.
• Strong clinical assessment and critical thinking skills required.
• Familiarity with Medicare guidelines and CMG classifications.
• Ability to work independently and manage multiple priorities.
• Excellent communication and documentation skills.
• Proficiency with EHR and MDS software systems required.

Responsabilités

• Oversee and coordinate the MDS/PPS program, schedule, and assessments for timely completion.
• Develop and coordinate nursing care plans based on patient needs and monitor timely completion by all departments.
• Analyze assessment data to identify complex patient needs and early clinical changes.
• Evaluate care plan effectiveness and update MDS/RAI within regulatory timelines.
• Collaborate with interdisciplinary teams to ensure compliant documentation.
• Maintain compliance with all regulations and facility policies.
• Perform additional duties to support resident care and facility operations.
