The Waters is seeking an experienced MDS Coordinator/Nurse to join our Indianapolis long-term care team. As an RN, you will conduct federally mandated resident assessments and compile data to influence care plans and reimbursement under RUG-IV guidelines.

Qualifications

• RN license required.
• MDS Certification (AANAC) optional but preferred.
• Experience with OBRA/PPS/MDS processes is a plus.

Responsibilities

• Perform federally mandated MDS assessments for residents.
• Coordinate OBRA schedule and MDS submissions.
• Develop and update care plans with interdisciplinary teams.
