Job summary

Infinity Healthcare Consulting is seeking an MDS Coordinator to perform federally mandated assessments at a long-term care facility. The RN will collect data, compile assessments, determine functional status and guide care plan development.

The role requires coordinating OBRA schedules, MDS timelines, and interdisciplinary care planning to optimize reimbursements and resident outcomes. A strong knowledge of CMS guidelines is essential.

Qualifications

• Registered Nurse (RN) is required.
• Optional: MDS Certification - AANAC is a plus.

Responsibilities

• Complete OBRA, PPS and MDS per the regulatory schedule.
• Coordinate Care Plans with the interdisciplinary team for timely initiation and revisions.
• Maintain monthly OBRA calendar and weekly OBRA, PPS and Care Plan schedules.
• Transmit OBRA/PPS MDS Assessments to CMS per guidelines.
• Coordinate and conduct Care Plan Conferences within 21 days of admission and every 90 days.
• Oversee MDS interviews (BIMS, PHQ-9, Pain, Activity) to completion and sign-off.
• Assist with Ancillary Departmental Assessments and ARD documentation.
• Support end-of-month billing and review for Triple Check.
• Monitor EMR data (ADLs, mood/behaviors) and update POC accordingly.
• Prepare and review MDS documentation for Medicare Part A and Managed Care.
