American Senior Communities' Riverwalk Village, located in Franklin, Indiana, seeks an experienced MDS Coordinator (RN) to oversee the RAI process and interdisciplinary care planning while ensuring regulatory compliance. The role emphasizes clinical assessment, team leadership, and compassionate resident care in a long-term setting.

The ideal candidate will have a BSN (preferred), at least one year in nursing management in long-term care, and two years of relevant nursing experience. C.A.R.E.

Qualifications

• Graduate of an accredited nursing program (BSN preferred).
• Minimum of one year in nursing management in long-term care.
• Two years of professional nursing experience in long-term care, acute care, restorative care or geriatric nursing.

Responsibilities

• Coordinate and complete the Resident Assessment Instrument (RAI) process.
• Lead interdisciplinary care planning and team collaboration.
