American Senior Communities is seeking a qualified MDS Coordinator (RN) for Brownsburg Meadows in Indiana. This role coordinates and completes the Resident Assessment Instrument (RAI) and the interdisciplinary care planning process while ensuring regulatory compliance.

Qualifications

• Graduate of an accredited school of nursing, preferably BSN.
• Minimum of one year in nursing management in the long-term industry.
• Two years of professional nursing experience in long-term care, acute care, restorative care or geriatric nursing setting.
• Demonstrates C.A.R.E. values to our residents, family members, customers and staff.

Responsibilities

• Coordinate and complete the Resident Assessment Instrument (RAI) and care planning process.
• Ensure compliance with state and federal regulatory requirements.
• Collaborate with interdisciplinary care teams.
• Lead and motivate staff to follow RAI processes.
