Job summary

Waters Of Indianapolis is seeking a qualified MDS Nurse (RN) to join our long-term care team. The role requires performing federally mandated MDS assessments and coordinating care plans with interdisciplinary teams to determine patient needs and reimbursement under CMS guidelines.

The MDS Coordinator will handle OBRA, PPS, calendar management, and MDS documentation, ensuring timely assessments and care planning.

Qualifications

• Must be a Registered Nurse (RN) with active licensure.
• Optional MDS Certification (AANAC) preferred but not required.

Responsibilities

• Complete OBRA, PPS and Managed Care MDS.
• Complete Admit MDS Tracking, Death in Facility tracking and discharge assessments per the RAI Manual.
• Coordinate Care Plans with other disciplines and ensure timely initiation or revision.
• Schedule Care Plan Conferences within 21 days of admission and every 90 days thereafter.
• Maintain OBRA calendar and weekly OBRA, PPS and IDT schedules.
• Transmit OBRA/PPS MDS to CMS per guidelines.
• Coordinate MDS interviews and ARD deadlines.
• Manage Insurance/Medicare/Managed Care caseload and re-authorizations.
• Oversee end-of-month billing and Triple Check processes.
• Review EMR data for ADLs, mood/behavior and documentation in ARD period.
