Job summary

OPCO Skilled Management in Indianapolis seeks an experienced MDS Coordinator to drive timely completion of the MDS process from admission to discharge. You will collaborate with the interdisciplinary team to assess residents, identify needs, and craft compliant care plans.

The role emphasizes meticulous documentation, adherence to MDS 3.0, PPS, and OBRA regulations, and proactive communication with staff and families.

Qualifications

• RN or LPN licensed in the state of practice.
• Minimum 2 years' nursing experience in a Skilled Nursing Facility.
• Strong knowledge of MDS 3.0, Medicare PPS, and OBRA regulations.
• Experience with care planning and quality indicators.
• Excellent attention to detail and communication skills.

Responsibilities

• Collaborate with the Interdisciplinary Team to complete MDS assessments and care plans.
• Ensure accurate, timely MDS documentation, coding, and record reviews.
• Coordinate daily stand-up meetings and review care/assessment schedules.
• Stay compliant with federal/state requirements and certification guidelines.
• Submit resident data to state and federal agencies with complete documentation.
