The MDS Nurse will lead the MDS process, organize care plans with the interdisciplinary team, and maintain accurate documentation to drive quality outcomes and resident wellbeing.

Formación

• Graduate of an accredited school of nursing.
• Valid RN or LPN license in good standing per state and federal requirements.
• Strong understanding of MDS process and nursing principles.
• Ability to coordinate interdisciplinary care plans.

Responsabilidades

• Lead MDS process for the facility per RAI guidelines.
• Coordinate and complete resident assessments and documentation timelines.
• Collaborate with nursing leadership to support care plans and quality initiatives.
• Attend meetings and oversee PPS/OBRA MDS activities daily.
• Maintain compliant MDS schedule and ensure accuracy of records.
