Job summary

OPCO Skilled Management seeks an MDS Coordinator to support the Director of Nursing and the RN Assessment Coordinator. You will oversee timely MDS submissions, CAAs, and the interdisciplinary care plan within a skilled nursing setting.

Requirements include RN/LVN licensure with experience in MDS, OBRA, and Medicare/Medicaid reimbursement. Strong documentation and communication skills are essential for regulatory success.

Qualifications

• RN or LVN license required
• Experience with MDS, CAAs, and interdisciplinary care planning preferred
• Understanding of OBRA regulations and MDS data set

Responsibilities

• Assist regulatory compliance with Federal and State regulations and certification
• Participate in survey process and correction of deficiencies
• Report trends from audits to the Quality Assurance Committee
• Manage the RAI process from MDS through the interdisciplinary care plan
• Initiate and monitor RAI process tracking and Medicaid forms in PointClickCare
• Ensure documentation standards are followed by staff
• Complete patient assessments and data collection
• Determine the Assessment Reference Date reflecting patient care needs
• Submit MDSs to State and track validation errors
