Job summary

Horizons Care Center is seeking an experienced MDS Coordinator to lead the Resident Assessment Instrument (RAI) process and ensure accurate, timely MDS assessments and care plans. This role supports regulatory compliance, reimbursement accuracy, and quality improvement by collaborating with the interdisciplinary team.

The MDS Coordinator will analyze documentation, drive improvements in assessment practices, and utilize available systems to support optimal clinical and financial outcomes aligned

Qualifications

• Current licensure in Colorado as a Registered Nurse.
• Minimum of two years direct MDS experience in a skilled nursing facility (SNF).
• RAC-CT certification required or must be obtained within six months of hire; RAC-CTA within twelve months of hire.
• Strong ability to create, build, and maintain productive relationships at all levels.

Responsibilities
