Horizons Care Center in Delta, CO is seeking a qualified MDS Coordinator to lead the RAI process, ensure accurate MDS assessments and care plans, and support regulatory compliance. Schedule: Mon–Fri 8am–5pm; competitive benefits and growth opportunities. This role sits within the Volunteers of America network.

As part of VOAN S, you will collaborate with an interdisciplinary team to drive quality improvement and optimize clinical and financial outcomes using available systems and resources.

Qualifications

• Must be licensed in Colorado as a Registered Nurse.
• Minimum of two (2) years of direct MDS experience in a skilled nursing facility (SNF).
• RAC-CT certification or must be obtained within six (6) months of hire; RAC-CTA within twelve (12) months.

Responsibilities

• Lead and coordinate the Resident Assessment Instrument (RAI) process for MDS assessments and care plans.
• Support regulatory compliance, reimbursement accuracy, and quality performance.
• Collaborate with the interdisciplinary team to drive continuous improvement in assessment and care planning practices.
• Fully utilize systems and resources to support clinical and financial outcomes aligned with organizational priorities.
