Horizons Care Center is seeking a driven MDS Coordinator (RN) to lead the RAI process and ensure accurate, timely MDS assessments and care plans. You will work with an interdisciplinary team to support regulatory compliance and optimize reimbursement.

Qualifications

• Current licensure in Colorado as Registered Nurse.
• 2+ years direct MDS experience in SNF with knowledge of PDMP, case mix systems, and reimbursement.
• RAC-CT and RAC-CTA certifications required or to be obtained within 6–12 months.

Responsibilities

• Lead and coordinate the Resident Assessment Instrument (RAI) process.
• Ensure accurate, timely, and compliant MDS assessments and care plans.
• Support regulatory compliance, reimbursement accuracy, and quality performance.
• Collaborate with interdisciplinary team, analyze docs and data, drive improvement.
