Job summary

Horizons Care Center is seeking an MDS Coordinator to lead the RAI/ MDS process, ensuring accurate assessments and care plans in a compliant, timely manner. You will collaborate with the interdisciplinary team to support regulatory and reimbursement goals.

Requires Colorado RN license, 2+ years of MDS in SNF, and RAC-CT/CTA certification within specified timeframes; strong relationship-building skills are essential.

Qualifications

• Colorado RN license active and in good standing.
• Minimum of two years' direct MDS experience in a skilled nursing facility.
• RAC-CT certification required or must be obtained within six months of hire; RAC-CTA within twelve months.
• Demonstrated ability to build relationships across all levels.

Responsibilities

• Lead and coordinate the RAI/MDS process for accurate, timely assessments and care plans.
• Ensure regulatory compliance and reimbursement accuracy through interdisciplinary collaboration.
• Analyze documentation and data to drive continuous improvement in assessment practices.
• Utilize systems and resources to support clinical and financial outcomes.
• Perform other duties as required.
