Job summary

Canyon Vista Post-Acute in Las Vegas, NV is seeking an experienced nurse leader to oversee the MDS and PDPM driven resident assessment process. You will coordinate weekly PDPM meetings, ensure timely MDS submissions, and supervise staff to maintain regulatory compliance.

You will train new MDS coordinators, monitor care plans, and collaborate with interdisciplinary teams to optimize resident outcomes. Strong data analysis skills and proficiency with Microsoft Office are required.

Qualifications

• BSN preferred and current RN license.
• Maintain licensure with State Board.
• Experience with MDS and PDPM required.

Responsibilities

• Oversee PDPM meetings for skilled patients.
• Track PDPM tools usage and data analysis.
• Monitor QM & QASP performance and provide recommendations.
• Provide PDPM and MDS training to new MDS Coordinators.
• Assess health status and level of care for new admissions.
• Ensure timely completion of all MDS Assessments including PPS Medicare.
• Maintain accurate MDS schedule and data entry.
• Verify electronic submissions of MDS and maintain records.
• Coordinate interdisciplinary MDS participation within regulatory time frames.
