Job summary

Forest Parking Nursing Center is seeking an experienced MDS Coordinator to oversee the RAI and care management processes from admission to discharge in compliance with company policy and federal/state guidelines.

Under the direction of the Director of Nursing and VP of Clinical Reimbursement, you will coordinate information systems operations and education for the clinical department while collaborating with the Interdisciplinary Team to ensure timely MDS assessments.

Qualifications

• Graduate of an approved RN/LVN program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility.
• Experience with MDS 3.0 and Medicare/Medicaid reimbursement.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Ensure decisions on completing or not completing MDS assessments follow clinical criteria per the RAI User’s Manual.
• Coordinate daily stand-up meetings, review resident care, and set assessment reference dates.
• Ensure compliance with federal and state regulations regarding MDS process completion and coordination.
• Monitor MDS and care plan documentation to support MDS coding in medical records.
• Maintain current MDS status per state and federal guidelines; ensure accurate data entry into MDS systems.
• Complete accurate coding of the MDS using medical records review, observations, and interviews.
• Other duties may be assigned as needed.
