Job summary

Forest Park Nursing Center is seeking an experienced MDS Coordinator to complete RAI and care management processes from admission to discharge in alignment with policy and federal/state guidelines. This role reports to the Director of Nursing and VP of Clinical Reimbursement.

You will coordinate MDS timing, ensure accurate documentation, and support the clinical team in delivering quality resident care in a fully staffed facility.

Qualifications

• RN/LVN licensure in state of practice.
• Minimum 2 years nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Understanding of OBRA regulations and the MDS process.
• Experience with MDS 3.0 preferred.

Responsibilities

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Decide when to complete additional MDS assessments based on clinical criteria.
• Coordinate daily stand-up meetings to review resident care and assessment dates.
• Ensure compliance with federal and state regulations for the RAI process.
• Maintain MDS status and accurate data entry in MDS programs.
• Accurately code MDS using medical records, observations, and interviews.
• Perform other duties as assigned.
