Job summary

Casa Arena Healthcare is seeking a skilled MDS Coordinator to oversee timely MDS and care management from admission to discharge in line with agency and federal guidelines.

You will work with the Director of Nursing and VP of Clinical Reimbursement to coordinate MDS processes, ensure accurate documentation, and monitor coding accuracy across resident records. RN/LVN licensed nurses with 2+ years SNF experience are preferred.

Qualifications

• Graduate of an approved RN/LVN program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility.
• Excellent knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Thorough understanding of OBRA regulations and MDS basics.
• Knowledge of the care planning process.
• MDS 3.0 experience preferred.

Responsibilities

• Collaborate with Interdisciplinary Team to schedule MDS assessments and care plan reviews.
• Decide on completing additional MDS assessments based on clinical criteria.
• Coordinate daily stand-up meetings reviewing resident care and assessment dates.
• Ensure compliance with federal and state regulations for the RAI process.
• Monitor MDS and care plan documentation in medical records.
• Maintain current MDS status per state and federal guidelines.
• Ensure timely data entry into computerized MDS systems.
• Accurately code MDS from chart review and interviews with staff, residents, families.
• Handle other duties as assigned.
