Forest Park Nursing Center is seeking an experienced and friendly MDS Coordinator to complement our fully staffed facility. You will oversee timely and accurate completion of the RAI and care management processes from admission to discharge in accordance with policy, guidelines, and state/federal regulations.

Under direction from the Director of Nursing and VP of Clinical Reimbursement, you may coordinate information systems operations and education for the clinical department, ensuring

Formación

• RN or LVN license required.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of Case-Mix, Medicare PPS and Medicaid reimbursement.
• Understanding of OBRA and Minimum Data Set regulations.
• Experience with MDS 3.0 preferred.

Responsabilidades

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Determine whether to complete additional MDS assessments based on clinical criteria.
• Coordinate daily stand-up meetings and review resident care and assessment reference dates.
• Ensure compliance with federal and state MDS regulations and documentation.
• Maintain current MDS status and accurate data entry in MDS programs.
