Job summary

Forest Park Nursing Center is seeking an experienced MDS Coordinator to support our fully staffed facility. You will collaborate with the Interdisciplinary Team to coordinate MDS assessments, care plan reviews, and regulatory compliance, ensuring accurate coding and documentation.

The role requires RN or LVN licensure, at least 2 years in a Skilled Nursing Facility, knowledge of OBRA, Medicare PPS, and the MDS 3.0 process. Join our team and help improve resident care and compliance.

Qualifications

• Graduate of an approved RN/LVN program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility.
• Strong knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Understanding of OBRA regulations and the Minimum Data Set (MDS).
• Experience with MDS 3.0 is preferred.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Decide on completing or not completing additional MDS based on clinical criteria.
• Coordinate daily stand-up meetings and review resident care and assessment dates.
• Ensure compliance with federal/state regulations for the RAI process.
• Monitor MDS and care plan documentation in medical records.
• Maintain current MDS status per state/federal guidelines.
• Enter resident information accurately into MDS software and complete coding.
• Other duties may be assigned as needed.
