Job summary

Life Care Center of Nashoba Valley is seeking an RN MDS Coordinator to lead the interdisciplinary completion and submission of MDS assessments, CAAs and care plans in line with CMS RAI guidelines.

The role requires an RN license, at least two years of nursing experience (geriatric preferred) and CPR certification. Full-time, Monday–Friday with on-site duties and a manager-on-duty rotation are included.

Qualifications

• Associate’s or bachelor’s degree in nursing from an accredited college.
• Active RN license in good standing.
• Two years of nursing experience; geriatric experience preferred.
• CPR certification available at hire or during orientation.

Responsibilities

• Coordinate and assist with completion and submission of MDS Assessments, CCAs, and Care Plans.
• Communicate CMS RAI requirements and ensure accuracy and timeliness.
• Review and interpret CMS QIES and post-acute care regulations as they apply to documentation.
• Attend and participate in routine care planning and interdisciplinary team meetings.
• Maintain confidentiality and adhere to Life Care’s policies and code of conduct.
