Job summary

Trilogy Health Services, LLC in Vincennes, IN is seeking an MDS Coordinator (LPN, RN) to oversee the resident assessment and care planning process and ensure compliance with federal and state regulations related to Medicare/Medicaid reimbursement.

You will conduct MDS assessments, develop care plans, provide information on Medicare/Medicaid options, and support care planning during discharge, while maintaining regulatory compliance and high-quality resident care.

Qualifications

• Current, valid state LPN or RN license.
• Three to five years’ experience in MDS or assessment in senior residential care or long-term care.
• Current, valid CPR certification required.

Responsibilities

• Conduct and complete the MDS assessment to evaluate residents’ status and triggers.
• Evaluate each resident’s condition to determine need for additional assessments or changes to the admission assessment.
• Prepare and electronically transmit timely MDS reports to national Medicare/Medicaid databases.
