Qualifications

• Must have and maintain a current, valid state LPN or RN license.
• Three to five years' experience in the MDS or assessment role in a senior residential care environment preferred.
• Current, valid CPR certification required.

Responsibilities

• Conduct and complete the Minimum Data Set (MDS) assessment.
• Evaluate each resident's condition and pertinent medical data.
• Prepare and transmit reports to Medicare and Medicaid databases.
• Develop written plans of care for each resident.
• Inform residents/families on available Medicare/Medicaid assistance programs.
