Job summary

Trilogy Health Services, LLC is looking for an MDS Coordinator (LPN, RN) to oversee resident assessments and care planning in Delphi, Indiana. The role involves conducting assessments, preparing reports, and developing care plans while ensuring compliance with regulations.

Qualifications

• Must maintain a current, valid state LPN or RN license.
• Preferred 3-5 years of experience in MDS or assessment roles.
• Current CPR certification is required.

Responsibilities

• Conduct and complete the Minimum Data Set (MDS) assessment.
• Evaluate residents’ conditions and relevant medical data.
• Prepare and transmit timely reports to Medicare and Medicaid.
• Develop written care plans for residents.
• Provide information on Medicare/Medicaid financial assistance.
• Ensure MDS notes accurately reflect services provided.
• Assist with discharge care planning.
