Job summary

You will conduct MDS assessments, evaluate resident conditions, and prepare plans of care, communicating with families about financial assistance and benefits. Requires LPN or RN license and CPR certification, with 3–5 years in MDS or assessment roles in long-term care.

Qualifications

• Must have and maintain a current, valid LPN or RN license.
• Three to five years' experience in MDS or assessment role in senior care or long-term care.
• Current, valid CPR certification required.

Responsibilities

• Conduct and complete the MDS assessment to evaluate residents' status and trigger CAA activities.
• Evaluate each resident's condition and data to determine need for additional assessment or amendments to the admission assessment.
• Prepare and electronically transmit timely reports to national Medicare/Medicaid databases.
