Job summary

Trilogy Health Services, LLC is seeking an MDS Coordinator (LPN/RN) in Valparaiso, IN to oversee resident assessments and care planning, ensuring regulatory compliance for MDS and Medicare/Medicaid reimbursement.

The role requires 3–5 years of MDS/assessment experience in long-term care, a current LPN or RN license, and CPR certification. You will work with the team to improve resident outcomes and care plans.

Qualifications

• Must have and maintain a current, valid state LPN or RN license.
• Three (3) to five (5) years’ experience in MDS or assessment in senior care or long-term care.
• Current CPR certification required.

Responsibilities

• Conduct and complete the MDS assessment to evaluate residents’ status and triggers.
• Evaluate residents’ condition to determine need for special assessments or changes to admission assessments.
• Prepare and electronically transmit timely reports to Medicare/Medicaid databases.
• Develop written care plans identifying problems, goals, and expected outcomes.
• Provide information on Medicare/Medicaid and other financial assistance to residents.
• Ensure MDS notes are descriptive of services and resident response.
• Assist with the care plan portion of discharge planning and participate in committees’ recommendations.
